REQUIRED PHYSICALS/MEDICATIONS IN SCHOOL/VACCINATIONS
REQUIRED PHYSICALS: NEW YORK STATE Education Law requires that all entering students and all students in Grades 2, 4, 7, and 10 receive a physical examination. An annual examination is also required for pupils receiving Special Education Services. Ideally, this should be done by your family’s health care provider. However, if this is not done, your child will receive a physical at school by the school physician at no cost. These will be given at the school as soon as the school physician is available.
If you are planning on having your family’s health care provider complete the physical, please contact the Health Office as soon as possible. Only those students who submit a complete written form from their family’s health care provider will be exempt.
MEDICATIONS IN SCHOOL: Just a reminder … If your child will need medications during school hours, the appropriate forms need to be completed and on file in the school health office by the beginning of the school year. If you have not received these forms, please call the school nurse.
VACCINATIONS—Parents of 5th Graders: In order to protect our children from vaccine preventable diseases, New York State Public Health Law Section 2164 was amended in 2004. The law requires that all students born on or after January 1, 1994, and who enroll in the 6th grade in any public, private or parochial school must provide documentation of being immunized against varicella (chicken pox).
VACCINATIONS—Parents of 7th Graders: Your child will NOT be able to enter 7th Grade without written proof of having received the Hepatitis B Vaccine, which is a series of 3 vaccines. Please contact your health care provider TODAY and set up an appointment if your child has not already been vaccinated.
